A North County nursing facility was slapped with the most severe penalty under state law after a patient death.
Aviara Healthcare Center in Encinitas received an AA citation and a $100,000 fine. The facility created a hazard by improperly storing a large mechanical patient lift in a hallway, according to the state health department.
A patient grabbed the lift during a fall, tipping it over. He later died from his injuries.
